Waiver Form

Thank you for choosing to participate in ball hockey with BHi London. This waiver form is a required part of the registration process for all players. Completing this form ensures that every player – and, where applicable, their parent or guardian – understands the nature of the sport and acknowledges the responsibilities and risks associated with organized ball hockey.

Ball hockey is a fast‑paced, accessible sport enjoyed by players of all ages across London, Ontario. To help us maintain a safe and well‑managed environment, please review each section of the waiver carefully and provide accurate information before submitting. Once completed, your registration can be processed and you’ll be ready to take part in the upcoming season.

Terms and Conditions

  • Fees paid for any season in the next twelve months (including tournaments and hourly rentals); include all games or times shown on the applicable schedule to be provided each season, as well as any subsequent playoff games.
  • The participant understands that he or she must supply his or her own stick, gloves, shin pads, elbow pads, helmet w/facemask, goalie equipment, jerseys, and jock or jill strap, as required by Ball Hockey International Inc.
  • Once the registrant has been assigned to a team, the registration fees are not refundable.
  • The parent, coach, and/or participant agree to abide by the rules of the game and rules of conduct, as set forth by Ball Hockey International Inc., in the BHI Rules and Guidelines, or BHI Coaches Guidelines. All BHI locations are True Sport facilities.

Release by Applicant

I, Hereby agree to abide by the terms and conditions set forth in this form for the duration of the next twelve (12) months, and in any other activities undertaken at BHI Brantford or BHI London, in which I may participate. I understand and assume all risks including COVID-19 and any other hazards incidental to my participation, including transportation to and from the activities and I waive, release, absolve of indemnity and agree to hold harmless BHI Brantford or BHI London, their respective officers, directors and employees, other participants, sponsors, the owner of the premises on which the BHI Brantford or BHI London facilities are located, persons transporting me to or from these activities, for any claim arising out of an injury or accident involving the applicant
